Format for Item Requests

There's certain information I need before I can make custom items.

First off, I need the basic info about what you want.
-Name (What do you want to call it?)
-Item Model (Please look on one of the following websites to find the model. In other words, you have to find an item already made and give me the name of that item so I can look it up in the system to pull up the Model ID. Please use something like Thottbot or WoWhead, Allakhazam, etc. If you leave it up to my discretion, I'm warning you ahead of time -- I have no artistic talent and will probably give you something obscure. )
-Type (What is it? Armor? Weapon? Food?)
-Subtype (What kind of armor, weapon, etc? Where do you put it, basically.)
-Stat block (What stats do you want on it?)

I'm going to use Derenas as an example.
